Role Description
Zócalo Health is seeking a skilled and empathetic RN Care Coordinator to provide clinical care coordination for patients receiving services from our interdisciplinary team of board-certified physicians, nurse practitioners, and licensed behavioral health therapists. This role also includes managing a panel of patients enrolled in clinical case management for chronic diseases, ensuring they receive comprehensive and coordinated care.
Additionally, the RN will triage clinical escalations, respond to urgent patient needs, and provide patient education to support self-management of chronic conditions. This role requires excellent organizational and clinical skills, as well as a passion for patient-centered care.

About the Role

Clinical Care Coordination

Serve as the primary clinical point of contact for patients, ensuring seamless coordination of care across medical and behavioral health services, both internally with Zócalo Health providers and externally with specialists and other providers.
Facilitate timely referrals to specialty care, diagnostic tests, or community resources as needed.
Work collaboratively with Zócalo Health physicians and nurse practitioners to support implementation of care.
Promote health literacy by using culturally and linguistically appropriate materials and communication strategies.
Support patients in navigating barriers to care, such as transportation or understanding care instructions.

Chronic Disease Case Management

Manage a dedicated panel of patients enrolled in clinical case management for chronic diseases such as diabetes, hypertension, and heart disease.
Conduct regular check-ins with patients to monitor condition management, medication adherence, and lifestyle modifications.
Develop and maintain detailed care plans to support achieving patient-led health related goals.
Provide education and coaching to empower patients to self-manage their chronic conditions effectively.
Educate patients and their families on health conditions, treatment plans, and available resources.

Clinical Escalation and Triage

Triage incoming clinical escalations, providing guidance to patients and collaborating with the care team to address urgent medical concerns.
Ensure appropriate documentation and follow-up for all escalations and interventions.

Team Collaboration and Documentation

Collaborate with the interdisciplinary care team to ensure patients receive comprehensive and coordinated care.
Accurately document all care coordination activities, clinical escalations, and patient interactions in the electronic health record (EHR), ensuring compliance with organizational policies.
Participate in team meetings, case conferences, and quality improvement initiatives.
Requirements for the Role

Education and Licensure

Current and unrestricted Registered Nurse (RN) license in California.
Bachelor’s Degree in Nursing (BSN).

Experience

Minimum of 2 years of nursing experience in care coordination, case management, or chronic disease management.
Experience in culturally diverse or underserved communities preferred.
Familiarity with electronic health records (Athena preferred) and case management tools.
Fluency in both English and Spanish.

Skills and Competencies

Strong clinical knowledge and ability to triage and address medical concerns effectively.
Excellent communication and interpersonal skills.
Ability to manage multiple priorities in a fast-paced, patient-centered environment.
Experience working in an early stage start up environment.
