We’re looking for a mission-driven Director of Patient Enrollment and Experience to lead and scale our outreach and engagement efforts across both our Enhanced Care Management (ECM) and Palliative Care programs. This role oversees a growing team of outreach professionals who connect with prospective clients, educate them on available services, and guide them through the enrollment process with care and compassion.

You’ll be responsible for developing and executing strategies that drive patient acquisition, build trust with underserved communities, and ensure a seamless, person-centered onboarding experience. This position blends healthcare sales, marketing, and remote team management, making it ideal for a leader who thrives at the intersection of operations, mission, and impact.

This role requires the willingness and ability to work core business hours aligned to Pacific Standard Time.

What you'll do
Leadership & Team Management

  • Directly manage and support the Clinical Outreach and Community Engagement teams responsible for enrolling patients into ECM and Palliative Care programs.

  • Set clear performance goals and KPIs for outreach efforts, including call volume, contact rates, enrollment conversions, and engagement quality.

  • Provide ongoing coaching, support, and performance feedback to ensure team alignment with patient-centered goals and regulatory standards.

Patient Enrollment (ECM & Palliative Care)

  • Develop and lead strategies and workflows to identify and enroll eligible patients into both the Enhanced Care Management and Palliative Care programs.

  • Ensure outreach efforts align with eligibility criteria, clinical priorities, and program timelines.

  • Partner with clinical teams to facilitate smooth transitions from initial contact to care plan initiation.

Patient Engagement & Marketing

  • Oversee outreach campaigns that engage prospective patients across multiple channels (phone, text, mail, events, community partnerships).

  • Tailor messaging to reflect the needs of diverse populations and promote trust, relevance, and clarity in every interaction.

  • Monitor engagement metrics and adjust outreach strategies for continuous improvement.

Cross-functional Collaboration

  • Collaborate with Product, Data, and Compliance teams to optimize CRM workflows, patient tracking, and reporting.

  • Work closely with Care Management, Provider Relations, and Clinical Operations teams to ensure timely handoffs and high-quality patient experiences.

  • Represent the outreach and enrollment function in cross-departmental planning and strategy sessions.

Your qualifications

  • 5+ years of experience in healthcare operations, patient engagement, or enrollment, with 2+ years in a team management role

  • Direct experience overseeing or managing call center/outreach functions in a healthcare setting

  • Understanding of Enhanced Care Management, palliative care models, social determinants of health, and person-centered care principles

  • Track record of leading teams to meet or exceed enrollment targets

  • Familiarity with CRMs (Salesforce preferred), EHR systems (AthenaHealth), and communication tools (Slack, Zoom, Google Workspace)

  • Strong interpersonal and communication skills, with a collaborative mindset and high empathy

  • Bilingual (English/Spanish), preferred
