What you'll do

Leadership & Operations

  • Provide day-to-day management and oversight of Palliative Care Delivery teams, including workforce planning, staffing decisions, and achievement of departmental objectives

  • Lead interdisciplinary teams of Providers, RNs, Social Workers, and Medical Assistants, ensuring clinical excellence across all roles

  • Develop and implement program strategies, goals, and initiatives to sustain and expand palliative care services

Performance & Quality Management

  • Drive execution and achievement of department KPIs aligned with VyncaCare's strategic priorities

  • Develop, review, and maintain evidence-based palliative care policies and procedures

  • Conduct and facilitate documentation compliance audits while monitoring quality outcomes and leading improvement initiatives

  • Apply independent judgment to develop methods, techniques, and evaluation criteria that drive results

Clinical Oversight

  • Assess palliative care referrals based on patient and family needs to determine the appropriate level of care and services

  • Ensure delivery of coordinated, patient-centered care across the continuum

Collaboration & Culture

  • Partner with department leaders across the organization to foster cross-functional collaboration

  • Model organizational core values and ensure team members understand and embody the company's mission and culture

This is a remote position (MST/CST, preferred) with occasional travel required for job duties, training, clinic/hospital visits, professional conferences, company events, and other business needs.

Your experience and qualifications

Education & Licensure

  • Master of Science in Nursing (MSN)

  • Active, unrestricted Nurse Practitioner (NP) license required; ability to obtain California NP licensure within 90 days of hire (if unlicensed in CA), plus additional state licensure as needed.

  • Board-Certified Nurse Practitioner (ACHPN certification preferred)

Experience

  • 5-8+ years of progressive healthcare management experience in fast-paced practice environments

  • Demonstrated expertise in:

    • Leading interdisciplinary clinical teams

    • Driving continuous process improvement

    • Clinical education and staff development

    • Serious illness and palliative care management

Skills & Competencies

  • Proficiency in Salesforce and Google Suite applications

  • Exceptional organizational skills with proven ability to coordinate internal and external stakeholders and drive outcomes

  • Strong collaborative approach with ability to work effectively across functions

  • Self-directed, entrepreneurial mindset with the flexibility and resilience to thrive in a dynamic, high-growth environment

  • Excellent connectivity and ability to work effectively in a remote environment.
