Vesta Primary Care “VPC” is one of Vesta Healthcare's divisions that specializes in primary care. We focus on older adults with long-term home care needs. Our practice serves patients in need of chronic care management, often home-bound, who benefit from a primary care provider who makes house calls. In Massachusetts, we operate as both home-based primary care and host an in-person medical office in Duxbury. We help these individuals live happier, healthier lives by partnering with their aides and caregivers, as a key part of the care team. We use a combination of in-person and virtual care to provide primary care to our patients.

We seek team members who are passionate about caring for older adults and collaborating with their caregivers to provide a team-based approach to care. Our team members are patient-centered optimists who always focus on doing what’s best for patients and their caregivers. Our team goes the extra mile to ensure tasks related to patient care are complete, timely and thorough. We see ourselves as being here to improve the quality of life for caregivers and care recipients, allowing them to focus on the important things, like time with one another.

The ideal teammate would be able to:

• Function as the primary care provider for a panel of patients. Responsible for diagnosis, management, prevention and treatment of health conditions.
• Conduct visits both in person at the patient's home and virtually for both acute and chronic issues. Expectation is 5-6 visits per day when scheduled for field visits, 8-10 visits per day when scheduled for telemed.
• Travel to the communities of Quincy, Braintree, Milton, Weymouth, and south of Boston communities of Dorchester, Hyde Park, Mattapan, and Roslindale
• Conduct care coordination and recommend/identify cost effective research based treatment and intervention
• Utilize strong clinical skills in physical assessment and chronic disease management for at risk adults and apply member specific Care Management and individualized care planning
• Be comfortable with advanced care planning discussions with caregivers and members
• Serve as a consulting resource on care management practice as needed
• Attend meetings, training sessions and participates on committees as needed
• Possess a strong knowledge of clinical procedures, standards and quality control checks
• Possess a strong knowledge of medical conditions, interventions and treatment
• Provide members, caregivers and facility education
• Monitor the quality of member’s care and updates plan of care

Would you describe yourself as someone who is:

• A Certified Nurse Practitioner with a license in good standing in the state of Massachusetts (required)
• Able to obtain certification and licensure as a Nurse Practitioner in the state of New York (required and will assist in obtaining if needed)
• Master’s or doctoral degree from an accredited institution for nurse practitioners (required)
• Language skills, proficiency in Russian or Spanish (preferred)
• Medicare participation and ability to have the company bill for services on your behalf (required)
• Certification from ANCC (or equivalent) as an Adult, Family, Geriatric, and/or Acute Nurse practitioner (required)
• 2+ years of Nurse Practitioner Experience (required), qualified for independent practice in your licensed jurisdiction (preferred)
• 1+ years of telephonic triage or equivalent experience (required)
• 2+ years of clinical experience working with complex adult populations (required)
• Ability to practice independently with little clinical support (required)
• Comfort using technology like Google Suite, multiple EMRs, Slack (required)
• The ability to work remotely and has a private area with a computer in their home/workspace (required)
• Experience working in home care and/or family medicine, geriatrics (preferred)
• Experience working within a clinical team environment
• Strong organizational skills, including the ability to prioritize

Pay range is $125,000 - $130,000 annually. (The referenced salary range is based on the Company's good faith belief at the time of posting. Actual compensation may vary based on factors such as geographic location, work experience, market conditions, education/training and skill level).
