The Special Projects RNCM (RN Care Manager) is a critical member of the care team working collaboratively with market and shared services teams on assignments including, but not limited to special pharmacy outreach, retrospective risk adjustment, and assistance with care team activities. This position will be working closely with Chronic Kidney Disease (CKD), End Stage Kidney Disease (ESKD) and Congestive Heart Failure (CHF) patients and other co-morbidity disease by phone or via telehealth.
Responsibilities
- Create and manage Care plan creation for unengaged members
- Assist and support care coordination activities that include the medical, behavioral, pharmaceutical, and social needs of the patient, identifying gaps in care and barriers to good health.,
- Develop care plans based on assessment and predictive modeling; in conjunction with the patient, patient’s nephrologist & PCP (Primary Care Physician), and other members of the care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient.
- Manage patients during transitions of care to minimize avoidable readmissions.
- Assess the patient’s knowledge of their medical condition(s) and provide education and self-management support.
- Participate in ongoing educational and compliance programs, staff meetings, continuous quality improvement, and performance/protocol evaluations and development.
- Maintain all current nursing licensure by completing applications for renewal in a timely manner and by complying with all requirements for continuing education.
- Provide support to care teams to assist with member/business needs as they arise.
- Flexibility on working hours depending on project, extended work hours and/or Saturday

Qualifications
Required
- Current unrestricted compact Registered Nurse (RN) license.
- Minimum 3+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings
- Highly organized and able to manage time effectively
- Knowledge and experience to empower patients in self-management and shared decision making
- Self-starter with demonstrated flexibility, and critical thinking skills
- Effective written and verbal communication skills demonstrating respect and cultural awareness during interactions with patients
- Demonstrates an aptitude and willingness to learn new responsibilities
Preferred
- Experience working with vulnerable patient population
- Demonstrates competence using MS Office products, electronic medical records, and telecom devices
- Additional single state licensure (s) as applicable
