About the Role
Seen Health is building and scaling a PACE program designed to deliver proactive, high-quality care for frail seniors. Our provider network is a core part of care delivery: it must be high-performing, reliable, culturally competent, and operationally easy for our care teams and participants to use.
The Provider Network Specialist role is equal parts provider network operations and execution. We already have a provider network in place; the work now is to make the network work day-to-day—maximizing existing contracts, unblocking access as we expand into new service areas, and building clean workflows that help our teams move quickly. This means you will work across a broad set of provider types involved in PACE—including PCPs, specialists, facilities, and ancillary partners (DME, home health, transportation, dental/vision, labs/imaging)— as well as novel services that aligned with Seen Health’s culturally competent model. This is a hands-on, fast-paced, cross-functional role for someone who’s willing to roll up their sleeves, handle case-by-case triage, and then codify what works into repeatable processes.
Key Responsibilities
- Own Network Readiness & Reliability: Ensure contracted providers are operationally “ready” and remove friction that prevents participants from accessing care.
- Unblock Access in Priority or New Service Areas: Surface and prioritize network gaps by service line and geography; drive activation of new providers and services through outreach, onboarding coordination, and internal/MSO collaboration.
- Execute Routine Contracting & Amendments: Manage standard contracting workflows end-to-end
- Build Rate Benchmarking (v1): Conduct market rate research and maintain a lightweight internal benchmarking library to support consistent pricing decisions and negotiations.
- Provider Onboarding & Operational Setup: Guide providers through onboarding steps including documentation collection, credentialing processes (as applicable), roster submission, systems setup, and go-live coordination; ensure smooth handoffs to clinical teams.
- Provider Relations & Issue Resolution: Serve as a primary point of contact for providers and office/facility staff, triaging and resolving inquiries related to participation requirements, onboarding status, contract terms, roster/directory accuracy, operational workflows, and payment. Partner closely with internal stakeholders and MSO counterparts to route issues to the right owners, coordinate timely resolution, and ensure consistent communication back to providers.
- Site Visits, Orientation & Training: Conduct provider office/facility visits as needed; deliver new provider orientation and ongoing training to reinforce expectations, processes, and participant experience standards.
- Coordinate Cross-Functional Issue Resolution: Partner with internal teams to resolve complex issues end-to-end; ensure clear ownership, timelines, and closure.
- Track, Trend, and Codify: Document and track provider inquiries and network issues through resolution, analyze themes and root causes, and translate repeat solutions into clear SOPs, templates, and playbooks that reduce future friction. Partner with cross-functional teams to socialize and train on new workflows, then monitor adoption and effectiveness over time and iterate based on feedback and outcomes.
- Maintain Accurate Network Data: Keep provider contact lists, rosters, and key documentation up to date, ensuring high data integrity across internal systems and any external reporting needs.
- Support Market Expansion: Execute locally in Los Angeles / San Gabriel Valley while helping adapt network operations as Seen Health expands its catchment area.

Qualifications
- 2-7 years of experience in healthcare, managed care, or provider operations; experience in provider relations, provider network operations, provider contracting support, and/or provider credentialing/onboarding is strongly preferred.
- Comfortable using AI tools to streamline repetitive work and improve speed/quality with strong judgment and professionalism.
- Proficient in Excel/Google Sheets and common productivity tools; CRM experience is a plus.
- Able to work independently to focus on the right needs, with strong agency and minimal oversight.
- Fluency in Mandarin or Cantonese Chinese is a plus
Location
- Based in Los Angeles / San Gabriel Valley, with comfort conducting local provider visits as needed.
Salary & Benefits
- Target compensation: $70-140K; our range has flexibility based on candidate experience in leveling.
