About the role
At Seen Health, Population Health drives both our clinical and financial model, ensuring frail seniors receive proactive, high-quality, cost-effective care in our PACE program. As a pay-vider, we blend managed care strategy with provider-led execution, ensuring every participant receives the right care, at the right time, in the right setting.
The Strategy & Operations – Population Health role is equal parts strategy and execution, responsible for 0→1 designing and implementing key capabilities that optimize quality, cost, and care coordination. This is a fast-paced, cross-functional role requiring strong leadership, execution, and problem-solving skills to build population health capabilities from the ground up
Key Responsibilities
- Drive Strategy & Execution: Own and drive initiatives end-to-end, from strategy development to implementation, ensuring projects stay on track.
- Build Managed Care & Utilization Processes: Build and implement scalable programs across care coordination, care management, utilization management, and provider engagement (e.g. transitions of care, poly-pharmacy, quality, etc.)
- Enhance Provider Network & Preferred Partnerships: Strengthen relationships with PCPs, specialists, pharmacies, and community-based organizations to optimize participant access to high-quality care.
- Cross-Functional Collaboration: Work closely with clinical teams, enrollment, compliance, and health plan operations to ensure seamless service delivery.
- Solve Ambiguous Problems: Tackle operational challenges with a problem-solving mindset, using data and stakeholder insights to drive decision-making.
- Data-Driven Decision-Making: Leverage claims data, encounter data, and quality metrics to monitor performance, identify trends, and drive continuous improvement.
- Health Plan & Compliance Support: Partner with health plan operations and regulatory teams to ensure compliance with CMS and state guidelines.
- Support Rapid Growth: Adapt quickly to changing priorities and help scale operations as Seen Health expands.

Qualifications
- Bachelor's degree required; graduate degree preferred (MBA, MPH); clinician-turned-operator welcomed (e.g. RN, SW)
- 5+ years of work experience in healthcare operations, population heath, or managed care, preferably in value based care settings, health plan or provider networks
- Experience in MSO, clinically integrated network (CIN) operations, utilization management, risk adjustment, or medical group operations are a plus.
- Experience in a startup or fast-paced environment is strongly preferred.
- Strong operational, analytical, and problem-solving skills. Ability to develop business cases, ROI analysis, and financial analysis
- Excellent communication and stakeholder management skills.
- Ability to execute and deliver results in an ambiguous, evolving environment.
Salary & Benefits
- Salary: $150,000-$175,000
