• Schedule transportation for all PACE participants
• Respond appropriately to needs as identified by the interdisciplinary Care Team
• Ensure that the transportation is on schedule and timelines are met
• Coordinate schedules, cancel schedules, and reschedule trips as needed
• Make adjustments to meet unanticipated changes and communicate those changes to the appropriate affected personnel
• Model and promote service excellence by developing and maintaining positive respectful relationships with PACE participants, families/caregivers, team members, and all staff throughout the organization
• Maintain all records for transportation, and ensure established policies and procedures are communicated and followed
• Satisfy all pre-hire requirements, including a physical examination, TB test, standard drug screen, and background / OIG checks
• Perform other duties as assigned

Qualifications

• 1 year experience with transportation scheduling and routing
• Some experience working with the elderly population is desirable
• Knowledge of the functions and responsibilities of a passenger driver
• Ability and skills to effectively plan and organize assignments in the most efficient manner
• Ability to meet FHCSD driving requirements and means to travel as needed in a timely manner
• Proficiency in English and spoken Mandarin and/or Cantonese

1. Salary: $18.00 – $24.00/hr
