The PACE Center Intake Coordinator is responsible for facilitating the enrollment of participants into the PACE program, ensuring compliance with program regulations, and coordinating care between various healthcare providers.

Key Responsibilities:

  • Coordinate and conduct initial intake screenings with potential PACE program participants, including determining eligibility and collecting necessary information for enrollment

  • Coordinate with various healthcare providers to ensure seamless transition of care for participants

  • Maintain accurate and complete participant records, including medical history, care plans, and medication lists

  • Work with interdisciplinary team to develop individualized care plans for each participant

  • Provide education and support to participants and their families regarding the PACE program and available resources

  • Ensure compliance with program regulations and standards

  • Participate in quality improvement initiatives and maintain knowledge of current healthcare regulations and trends

Key Qualifications:

  • Bachelor's degree in healthcare, social work, nursing or related field preferred

  • Minimum of two years of experience in healthcare service coordination or case management required

  • Knowledge of Medicare and Medicaid regulations preferred

  • Excellent communication and interpersonal skills

  • Ability to work independently and as part of an interdisciplinary team

  • Strong organizational and time-management skills

  • Ability to maintain confidentiality and exercise good judgment

  • Proficiency in Microsoft Office and electronic health records (EHR) systems
