We are looking for an experienced Registered Nurse/Physician Assistant/Nurse Practitioner with prior experience in clinical care with an interest in developing clinical quality programs, providing insights and supporting our clients with adoption and integration. The ideal candidate has a strong understanding of the educational needs of Primary Care Providers; ability to identify high impact solutions to barriers to care; experience interfacing with providers and clinic leadership effectively and a strong understanding of clinical practice. Responsibilities will include:

  • Review patient clinical charts to initiate comprehensive patient-centric eConsult submissions to specialists
  • Review and analyze specialist consultant responses escalating any clinical quality concerns to Clinical Operations team
  • Synthesize specialist responses with patient information to develop clinically appropriate care plans; leveraging strong understanding of disease states and guideline directed care within clinical practice
  • Coordinate and follow up with necessary parties to ensure proper implementation of care plans interfacing with specialist panel and clients to optimize PCP utilization
  • Maintain accurate and up-to-date records of eConsult submissions and follow-up interactions
  • Analyze population health data provided by clients to understand patient outcomes and identify areas for improvement
  • Respond to requests from clients to ensure satisfaction and address any concerns

Desired skills and experience:

  • 1-3 years’ experience as a practicing healthcare provider (NP/PA/RN) with understanding of clinic workflow and disease management
  • Knowledge of healthcare systems, terminology, and disease management programs
  • Experience with electronic medical records and eConsult platforms
  • Strong organizational and communication skills
  • Ability to work independently and manage multiple tasks simultaneously
  • Active clinical license required; clinical degree such as NP/PA/RN required

This is a part-time or contractor position that may require some flexibility in schedule. The hourly rate for this part-time contract role is $50.
