About the Role:

We are seeking a dedicated and autonomous Per Diem Nurse Practitioner (NP) to provide high-acuity in-home care to patients across Brooklyn. This unique role blends the fast-paced nature of urgent care with the personalized approach of house calls, offering you the autonomy to practice at the top of your license. You'll work independently with comprehensive support, drive a fully stocked company vehicle, and make a tangible difference in people's lives every day.

What You'll Do:

• Perform comprehensive in-home clinical assessments and focused physical exams.
• Administer acute treatments, including IV fluids, advanced wound care, splinting, and injections.
• Order and interpret diagnostic tests such as labs, EKGs, and bedside diagnostics (e.g., POCUS).
• Provide vital patient and caregiver education and empowerment.
• Coordinate seamlessly with multidisciplinary teams and contribute to patient-centered care planning.
• Deliver care with a compassionate, palliative, and patient-centered philosophy.

Who You Are:

• Currently a Licensed New York Nurse Practitioner (NP) with BLS certification (ACLS a plus).
• Possess 2+ years of experience in high-acuity settings such as urgent care, emergency department (ED), or hospital medicine.
• Thrive in independent practice and remain calm and effective in unexpected situations.
• Passionate about delivering high-quality, patient-centered care in a home environment.
• Flexible and available for varied part-time shifts, including some evenings, weekends, and holidays.
