myLaurel, a leading provider of home-based acute care, is seeking experienced Hospitalist Nurse Practitioners (NPs) in the Greater New York City Area. Our dedicated team provides acute medical care to patients in their homes to avoid ER visits, hospital admissions, and post-acute care stays.

Competitive Pay and Benefits: This full-time Hospitalist NP role offers a salary of $149,000 to $165,000 annually, plus a sign-on bonus. There is a robust benefits package including CME reimbursement, tuition reimbursement, 401k, company-sponsored life insurance, medical, dental, vision, paid parental leave, sick leave, and discretionary time off. A company vehicle is also provided for patient visits!

As a member of the Clinical Delivery team, you will be a part of a team that is bringing patient care directly to the home, while ultimately improving outcomes, patient experience and reducing overall cost of care. We are looking for motivated, creative individuals to join our team and help us create a new standard of care!

In this role you will work 36-40 hours scheduled per week, with some weekends and holidays included. Shifts will be 8-10 hours, and we will consider part-time availability.

Hospitalist NP Key Responsibilities:

  • Conduct comprehensive acute care assessments of patients in their homes, mirroring hospital-level care, This includes evaluation of past medical history, review of systems, health screenings, medication reconciliation, vital signs, and focused physical exams.
  • Identify and manage urgent and emergent situations and intervene appropriately in a home setting, initiating appropriate interventions.
  • Order, perform, and analyze diagnostic tests commonly utilized in acute care settings, including labs, EKGs, and point of care ultrasounds.
  • Administer hospital-level therapeutic interventions, such as IV fluids, IV/IM/PO medications, and other treatments as needed for acute conditions.
  • Coordinate diagnostic testing and follow-ups.
  • Integrate patient goals into care plans, including palliative principles.
  • Educate patient on topics such as disease process, medication, and compliance
  • Communicate with referring clinicians and ensure transition of care post-visit.
  • Support clinical guidelines, pathways, and quality initiatives.

Hospitalist NP Requirements

  • Current State Licensure as an NP in New York.
  • Minimum two years of experience in relevant areas working as a Nurse Practitioner in acute care settings (e.g., ED, Urgent Care, Hospital).
  • Experience managing complex medical conditions and co-morbidities in an acute care setting.
  • Current BLS certification (ACLS preferred).
  • Physically ability to navigate stairs and complete patient care in home settings.
  • Eligibility to work in the U.S. without sponsorship.
