About the MedArrive Lead Social Worker Role:

Our Social Workers play a vital role at MedArrive to impact the lives of the patients and families in our community. The Lead Social Worker will care for a reduced panel of patients directly, while also taking on additional support roles including administrative duties, operational support, health plan relationship building, collaboration with Lead Field Providers, and coaching, education, and general support to our team of social workers.

What You’ll be Doing: General Social Work Duties

  • Responsible for an assigned patient panel by assessing and addressing social determinant needs for patients, in collaboration with the medical team
  • Educate patients and families to support improvement in health literacy
  • Research and identify community resources to connect patients with organizations to help with housing, food insecurity, employment, childcare, etc.
  • Collaborate and coordinate with the payer to help connect patients with medical care and to encourage follow-up with their PCP, chronic care Provider, and mental health treatment
  • Partner and work strategically across the MedArrive team to build a replicable Playbook and Implementation Plan for future programs
  • Incorporate feedback from daily operations to improve assigned patient panel outcomes as well as the initial program model
  • Maintain timely patient medical records and documentation consistent with state regulations and MedArrive standards
    Participate in continuing education as required by the state in which you are licensed
  • Other related duties as deemed necessary

What You’ll be Doing: Lead Social Work Duties

  • Work directly with Regional Clinical Director to support needs of the Social Work team as they relate to the Longitudinal programs while supporting a smaller panel of patients with direct SW care
  • Personal and team accountability for patient outcomes, monitoring program-specific metrics, and supporting improvement plans as needed, while encouraging and promoting efficiency
  • Social work team support including: routine SW Chart Audits to ensure documentation expectations are met by social work team, onboarding support for new hires, mentoring/coaching social work team, virtual ride-alongs, creating/developing workflow documents or job aides as needed
  • Administrative support including managing time off calendars/tracking attendance, monitor CEU funds expenditure, manages social work schedules
  • Health Plan partner support including facilitating internal rounds meetings, acting as point of contact for the health plan Case Management (CM) team, lead external CM meetings as requested by health plan
  • Other related duties as deemed necessary

Minimum Requirements:

  • Licensed Clinical Social Worker with active TX licensure
  • 3 - 5 years of experience providing social work/case management services
  • Experience and proficiency working with an Electronic Medical Record system
  • Ability to communicate and interact with Senior Leadership and external executive audiences
  • Excellent written, oral, and interpersonal skills to ensure that each of your interactions with patients, families, and team members is effective and meaningful

Preferred Requirements:

  • Experience in Clinical Social Work, Psychology, Public Health
  • Bilingual English/Spanish is preferred.

Skills & Abilities:

  • Energy and enthusiasm consistent with working for a startup; ability to self-teach in order to problem solve, takes initiative
  • Demonstrated understanding of community resources in Texas, with a particular focus on Houston and/or Dallas
  • Successful at patient engagement and utilizing motivational interviewing techniques
  • Successful track record of establishing credibility and trust with a diverse patient population
  • Strong critical thinking, creative problem solving, judgment, and client management skills
  • Ability to maintain confidentiality, tact, and diplomacy
  • Strong organizational skills; ability to remain organized and productive in a fast-paced work environment with competing priorities
