We are seeking a highly skilled and compassionate Collaborative Care Social Worker (LCSW) to join our growing New York team on a part-time basis! This role will provide care in a primary care clinic, virtually, and in community settings as needed. As a core member of our collaborative care team, you’ll work closely with partner primary care providers, a supervising psychiatric consultant, and our broader team in the field to ensure thoughtful, coordinated support for patients needing behavioral health care.

What You’ll Do

As the first social worker in New York, you’ll play a critical role in embedding behavioral health into primary care settings. You’ll lead mental health care coordination for patients while strengthening collaboration across the broader care ecosystem. Your deep clinical insight and relational strengths will help bridge gaps across the mental health continuum.

Key Areas You’ll Add Value

  • Deliver comprehensive social work services to patients and families: psychosocial assessments, brief individual/family counseling, advocacy, service linkage, group work, and long-term care planning.
  • Screen and assess for common mental health and substance use disorders; drive patient engagement and ensure follow-up care.
  • Provide education to patients on common mental health concerns and available treatment options.
  • Track treatment responses and monitor for changes in symptoms, medication side effects, and clinical complications.
  • In partnership with a supervising psychiatrist, physician, or nurse practitioner, support psychotropic medication management—monitoring adherence, side effects, and treatment efficacy.
  • Use evidence-based techniques like behavioral activation, problem-solving therapy, and motivational interviewing to deliver patient-centered behavioral interventions.
  • Refer patients to appropriate psychosocial treatments and community-based resources when clinically indicated.
  • Participate in regular case consultations with the psychiatric consultant to review care plans and recommend changes as needed.
  • Maintain consistent documentation of patient progress and outcomes in a registry system or electronic health record (EHR).
  • Facilitate referrals to external services including in-person behavioral health care when indicated, social supports, substance use programs, and vocational resources.
  • Collaborate with patients to develop relapse prevention and self-management plans as they approach discharge from care.

What You Bring

  • Master’s degree in Social Work (MSW).
  • Active, unrestricted New York Licensure (LCSW)
  • Experience and/or training in the Collaborative Care Model for behavioral health integration in primary care strongly preferred
  • Track record of success working as part of a multidisciplinary team in a patient-centered setting.
  • Comfort with technology and EHR systems—able to adapt to evolving tools and workflows.
  • Strong grounding in case management, conflict resolution, and care team collaboration.

Schedule: 16 hours per week, consistently scheduled based on your availability

Benefits: This is a part-time position and is not benefit-eligible

Compensation: This is a part-time, non-exempt position with a set schedule of 16 hours per week. The hourly rate is $44–$48, which is equivalent to $92,000–$100,000 on a full-time basis. Actual rate will be commensurate with candidate experience.
