We are looking for a Nurse Practitioner to join our Care Team in Michigan! As an integral part of the local care team this Nurse Practitioner role requires strong organizational skills, effective communication and a strong focus on supporting both patients and care team members across clinical and functional settings. The Nurse Practitioner will need a strong ability to problem solve patient cases and be familiar with local resources in the community to utilize in assisting patients with social determinants of health. The Nurse Practitioner will work closely with Social Workers and Health Coaches to provide support to patients.

What you’ll do

As a Nurse Practitioner here at Diverge Health you will serve as the lead clinician in the interdisciplinary care team, providing clinical supervision for Health Coaches and managing escalations. You will act as the clinical liaison between Health Coaches and our partnered primary care providers, ensuring seamless communication and collaboration.

• Providing clinical oversight and guidance to health coaches
• Identify and manage patient case escalations
• Oversee patient panels, identify trends and enhance care programs.
• Patient care coordination with primary care providers, addressing case escalations, and ensuring smooth team communication in a fast-paced environment.
• Creative problem solving to support the day to day of the health coaches.

What you bring

• 2+ years’ experience as a Nurse Practitioner in primary care
• Extensive experience utilizing EMRs and the ability to navigate multiple different EMR systems with ease
• Computer literacy skills
• Strong with technology/savvy
• Ability to lead interdisciplinary care teams/Experience working in this setting
• Strong communication skills
• Experience teaching and educating others on clinical concepts
• Chronic Condition Management experience
• Comfortable working in a hybrid model that includes remote work, in-office collaboration, in-clinic support at primary care practices, and in-home patient visits as needed
• Ability to travel locally on a weekly basis in and around Detroit to meet the team in the office and support practice visits

Preferred

• 2+ years' experience with Microsoft Suite
• Experience with Salesforce
• Value-based care experience

This is a full-time, exempt, salaried position. Commensurate on candidate experience, the expected base salary range for this role is $100,000-$140,000.
