We are looking for a Senior Manager of Practice Liaisons to join our team! This role is central to ensuring our provider partners have the support and structure they need to succeed in value-based care. You’ll guide a high-performing team across multiple markets, helping translate Diverge’s mission into strong local relationships and reliable operational execution. It’s a great fit for someone who can lead with both empathy and accountability, communicate clearly across teams, and bring focus to fast-moving, high-stakes environments.

What You’ll Do

As the Senior Manager of Practice Liaisons, you will oversee day-to-day practice-facing operations, supervise a team of Practice Liaisons across multiple Diverge Health markets (specifically Detroit and Cleveland), and play a vital role in supporting practice performance. Reporting to the Market President with direct oversight from the Senior Director of Practice Liaisons, this role fosters strong relationships with provider organizations and senior leadership while driving initiatives that align with Diverge Health’s mission and strategic objectives. Regular travel to markets will be required.

Key areas you'll add value:

  • Lead and oversee daily operations of the Practice Liaison team, ensuring high performance and exceptional support across markets.
  • Build and maintain strong relationships with provider organizations, IPAs, and senior market leadership, serving as a critical communication link.
  • Collaborate with market leaders and facilitate cross-functional coordination between teams, including Care Operations Directors, Health Coach Managers, and Network Development.
  • Stand up and operationalize new workflows in fast moving environments.
  • Analyze data from operational reports and dashboards to identify trends and communicate insights to stakeholders, in order to identify key areas of opportunity and improve physician network performance.
  • Partner with the National Medical Director of Network Performance and Market President to enhance physician engagement and satisfaction.
  • Provide strategic input during Joint Operating Committee (JOC) meetings to align efforts with network practices.
  • Support hiring, onboarding, and professional development of Practice Liaison team members using the Practice Liaison Playbook and other resources.
  • Drive effective change management strategies in collaboration with Market Presidents and Medical Directors.
  • Help facilitate communication and shared decision-making across a matrixed organization with multiple key stakeholders.

What You’ll Bring

  • Bachelor's degree in Healthcare Administration, Business, or related field.
  • 5+ years of experience in value-based care, including knowledge of HEDIS quality metrics and risk adjustment methodologies.
  • 3+ years of experience in a managerial or operational role within a healthcare setting, with direct team supervision.
  • Proven ability to translate strategy into action, navigate ambiguity and drive execution forward.
  • Reputation for delivering excellent service and managing senior-level stakeholder expectations.
  • General operations experience within health systems or clinics, including with EHR/EMRs, population health tools, and other healthcare platforms.
  • Strong understanding of primary care operations and organizational change management processes.
  • Demonstrated leadership in team development and performance management.
  • Proficiency in analyzing operational reports and utilizing data to inform decisions.
  • Advanced skills in MS Excel, including pivot tables and formula-based reporting;
  • Experienced in creating and delivering insight-driven PowerPoint presentations for executives and providers to communicate performance trends, operational updates, and strategic initiatives.
  • Ability to work effectively in a remote environment and manage multiple projects simultaneously.
  • Willingness to travel up to 50% of the time, if not local to the Detroit or Cleveland markets.

Preferred experience

  • Experience supervising teams across multiple markets in a hybrid environment.
  • Familiarity with Salesforce CRM.
  • Experience in a start-up or fast-scaling healthcare organization.

This is a full-time, exempt, salaried position. Commensurate on candidate experience, the expected base salary range for this role is $120,000-$135,000.
