We are seeking a mission-aligned Social Services Advocate to lead the development and implementation of our Benefits Enrollment program across multiple markets! This position plays a critical role in helping patients access essential Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI) benefits.

As a Social Services Advocate you will serve as a primary point of contact for patients navigating the disability application process, providing virtual guidance, application preparation, and documentation support. This role involves both direct patient engagement and internal collaboration with cross-functional teams including Market Operations, Population Health, Data, and Engineering to scale a high-impact, system-wide initiative.

Key areas you’ll add value:

  • Conduct structured virtual interviews with patients and caregivers to assess eligibility and collect necessary information for SSI/SSDI applications.
  • Request and compile medical documentation from relevant providers and submit completed applications to the Social Security Administration (SSA) and Disability Determination Services (DDS).
  • Serve as the designated SSA Representative for patients without legal representation and collaborate with external legal or advocacy partners when appropriate.
  • Partner with market-level operational teams to support physical document handling, assess eligibility criteria, and escalate patient safety or engagement concerns as needed.
  • Contribute to the development of standardized workflows, SOAR-based protocols, and tooling that enhance scalability, accuracy, and compliance within the Benefits Enrollment program.
  • Provide insights to cross-functional teams to guide product design, system improvements, and performance monitoring related to public benefit access.

What you’ll bring

  • Minimum of 2 years of experience in social work, case management, disability advocacy, or public benefit navigation, specifically with SSI/SSDI or related programs.
  • Current SOAR (SSI/SSDI Outreach, Access, and Recovery) certification at the time of hire.
  • Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related human services field preferred.
  • Experience supporting individuals with complex medical, psychological, or socioeconomic conditions.
  • Previous experience working in or in partnership with SSA, DDS/DDD agencies, or similar government institutions preferred.
  • Experience designing or optimizing benefit enrollment workflows or digital platforms preferred.
  • Proficiency in virtual engagement and outreach, including telephone and video-based interactions.
  • Demonstrated knowledge of SSA and DDS procedures, documentation standards, and application processing.
  • Strong verbal and written communication skills, with attention to accuracy and detail.
  • Familiarity with trauma-informed care, motivational interviewing, and recovery-oriented approaches.

This is a full-time, exempt, salaried position. Commensurate on candidate experience, the expected base salary range for this role is $70,000-$80,000.
