Cayaba Care is working to drastically change the experience of affected communities by reimagining how we deliver meaningful and compassionate care to expectant moms.

Cayaba Care’s mission is to improve pregnancy, birth and the postpartum journey for women and their families by reimagining how maternity care is delivered. Our team is focused on building trust-based engagement and wrap-around support for expectant moms at home, in person and virtually to ensure they have all the social support they need for a successful and joyful pregnancy journey. We help our members with care navigation, doula support, assistance with lactation, nutrition, housing, transportation and a wide range of other needs. We leverage technology to support our virtual engagement that allows us to meet all our moms where they are.

We ensure that our members have trusted sources for support, resources, and information throughout their pregnancy, delivery and well into postpartum. Cayaba Care does not replace the primary obstetrician or doctor’s relationships. Instead, we support expectant mothers with all other health-related social needs not typically available through their doctors’ offices.

The Role:

Lead day-to-day care delivery operations across Maryland and DC. This role is accountable for consistent execution of Cayaba’s care model, team performance, and operational efficiency across payer programs. You ensure teams are supported, aligned, and delivering against defined KPIs while partnering closely with the Director of Care Delivery on scale, stability, and continuous improvement. This role is remote, but does require regular travel across the Washington, D.C, and Maryland areas to shadow and train our team.

This role is central to operationalizing a unified care model as enrollment grows across markets.

Key Responsibilities

Care Delivery Operations and Performance

  • Own day-to-day execution of care delivery across assigned markets, ensuring consistent performance against engagement, visit completion, documentation, and member experience metrics.
  • Lead, coach, and support multidisciplinary care teams.
  • Ensure appropriate caseload distribution and staffing coverage aligned with geography and program design.
  • Monitor operational performance trends and identify risks or gaps requiring intervention
  • Build and maintain a high functioning clinical and non clinical care team across assigned markets.
  • Conduct virtual & in-person visit shadowing to identify training needs and develop relationships with the team
  • Oversee the recruitment, training, and ongoing evaluation of Care Team staff to ensure that new hires are well-prepared and aligned with our care standards

Operational Excellence

  • Drive consistent adoption of workflows, SOPs, and program updates across markets.
  • Identify operational friction points and partner with the Director of Care Delivery to implement solutions.
  • Maintain strong familiarity with payer program requirements to ensure reliable execution and compliance.
  • Build strong feedback loops between frontline teams and leadership to inform ongoing optimization.

Stakeholder and Market Support

  • Provide regular updates, risks, and insights to the Director of Care Delivery to support planning and growth.

Qualifications

Required

  • Registered Nurse, compact license - nice to have.
  • 5+ years leading healthcare operations or care, delivery teams across multiple sites or markets
  • 3-5 years of management experience in hybrid or remote clinical environments
  • Willingness to travel up to 30% for visit shadowing & team-building activities
  • Demonstrated ownership of staffing scheduling, throughput, and KPI performance
  • Experience operationalizing and standardizing care models at scale
  • Proven ability to lead multidisciplinary teams and drive consistency across roles.
  • Strong data literacy, with experience using performance metrics to inform action

Preferred

  • Experience in maternal health, community-based care, or Medicaid populations.
  • Clinical background or strong clinical fluency
  • Experience working in regulated healthcare environments.

Our compensation range for this position is between $100k-$108k. Exact compensation will depend on years of direct and relevant experience.
