We seek a passionate and experienced Care Manager with a background in maternal services, preferably those experienced in pregnancy and postpartum care. The ideal candidate will be willing to either work part-time or full-time, interfacing with pregnant patients with hypertensive disorders and/or mental health and emotional support needs. Thus they play a crucial role in our mission to revolutionize maternal care through remote patient monitoring. This remote role offers the opportunity to change how prenatal and postpartum care is delivered.

The Care Manager is an integral part of our operations team and is responsible for providing care coordination and health navigation services.

Key responsibilities include:

  • Empowering patients through motivational interviewing-based conversations
  • Patient education using evidence-based practices and guidelines
  • In-app messaging to support provider-driven care plans and health literacy
  • Monitoring patient data
  • Escalating to appropriate clinical resources as needed
  • Providing coordination between providers as appropriate
  • Scheduling of patient appointments

This individual will interact directly with patients and providers and work closely with other team members on the Care Management Services team at Babyscripts.

Our products aid in understanding and assessing risk early in pregnancy, providing the patient and care team with the information they need for the best care possible.

The base of the Babyscripts solution is myJourney, a mobile app with thousands of pieces of clinically validated content delivered at the right time in the mom’s pregnancy journey. Modules tailored to a patient’s risk level are built onto myJourney to provide the best-fit support for that patient while providing care providers with greater insight into the patient’s health and well-being.

Central to the Remote Patient Monitoring (RPM) program is the Babyscripts myBloodPressure, an intelligent, end-to-end remote monitoring solution with everything a pregnant person will need to manage blood pressure remotely under the watchful care of the care manager and a digital companion configurable for specific to risk levels, and an intelligent trigger system to optimize workflows designed for Obstetric caregivers.

Our digital platform and virtual patient advocates engage moms and help them connect to better care. We monitor patient-generated data and medical records to trigger personalized interventions while informing providers of addressable risks, allowing the healthcare team to work collaboratively. We are ushering in a new era for women’s health. This is a more innovative way to care.

Key Responsibilities

The Care Manager is a healthcare professional responsible for building trusting relationships with pregnant patients, driving positive health behavior change, and providing vital sign monitoring through the Remote Patient Monitoring (RPM) program. This remote role offers the opportunity to proactively monitor, educate, and coach patients on their journey through pregnancy and postpartum:

  • Conduct preventive health education and coaching sessions remotely, utilizing a care manager portal to monitor physiological markers prescribed by the patient’s Obstetric Care Provider, such as blood pressure and more.
    • Collaborate with patients to adhere to Hypertension Care Plans and define the provider's health goals.
    • Monitor patient readings and conduct timely follow-up for abnormal results.
    • Follow up with patients who have not submitted readings in several days.
  • Schedule and perform regular monthly real-time interactive telehealth visits, documenting findings in the Babyscripts care manager platforms, and schedule follow-up monthly encounters for RPM patients with uncontrolled hypertension, ensuring continuity of care.
    • Educate patients on proper techniques for monitoring vital signs and managing acute or chronic medical conditions.
    • Motivate patients to measure their blood pressure as frequently as their provider prescribes, given their health risk status and thus enrollment in the RPM program.
    • Explain the benefits to the healthcare provider team when such measurements are taken to better care for the patient.
    • Work with the patient to remove barriers to measure as prescribed, regardless of the barrier.
  • Manage alerts, answer routine patient questions, and elevate them as necessary to the practice's on-call clinical staff, ensuring a seamless patient experience.
    • Facilitate communication between patients and healthcare providers, ensuring a patient-centric approach to care.
    • Collaborate with higher-level providers by sending encounters related to patient communication or medication issues.
  • Provide detailed documentation of all patient encounters and communication, maintaining accurate records.

Required Experience

  • Experience in healthcare settings, particularly maternal (OB Office, Labor & Delivery, Postpartum, Doula, Lactation, etc.)
  • Proficient in using digital health platforms and technology.
  • Have a passion for maternal care.
  • Excellent communication and interpersonal skills.
  • Ability to work alone effectively and efficiently.
  • Detail-oriented with critical thinking abilities.
  • Comfortable sitting for extended periods.
  • Effective communication and customer service skills.
  • Passion for improving patient outcomes.
  • Bilingual or multilingual in English and any other spoken language (preference for Spanish).

Preferred Experience

  • Strongly Preferred: Degree in Nursing, Health Administration, or related field.
  • Strong understanding of prenatal and postpartum care protocols.
  • Previous experience in care management or remote patient monitoring.
  • Maternal-based certifications (i.e., DONA International, IBCLC, etc.)

Location

  • The candidate must be able to work remotely successfully and have experience designing and establishing a productive, efficient, and balanced work-from-home routine that does not detrimentally affect professional outcomes or personal life.
  • Occasionally, the candidate is required to attend in-person meetings, sessions, workshops, and other events with their team members.

Compensation & Benefits Information

  • All open positions have flexible hours, with some shifts after hours (5 pm to 10pm) M-F and rotating weekends (10 am—5 pm), four to five days a week. Schedules will be determined based on the candidate's availability.
  • Part-time or full-time positions are available with the flexibility to manage odd hours when patients are most accessible.
  • Part-time 1099 contractors and full-time W2 employees will be compensated for their time as logged in the Care Manager Portal, which will not be less than $23/ hour. Actual compensation packages are based on a wide array of factors unique to each candidate, including but not limited to skill set, years & depth of experience, and certifications.
