Amae Health is seeking the right Community Health Worker to partner with us in providing care for complex adult patients. This position is full-time, in-person and based out of our clinic in Houston.
About the Job
The Community Health Worker plays a critical role in establishing and sustaining life-changing relationships between Amae and our patients as you help support them through their care journey. This position is an in-person, patient-facing role. The Community Health Worker will work collaboratively with patients, co-workers, and the community. They will advocate on behalf of patients and help them navigate the health and social services systems in Houston. The Community Health Worker will work closely with patients to address problems, answer questions, gather and provide information and advice, and connect patients to resources and to the community. The Community Health Worker will meet patients in the hospital, in the community, and at times in their homes. They will initiate, establish, and maintain relationships with patients and their families while developing trust and rapport.
- Collaborate with a multidisciplinary team to support patients in their care journey.
- Advocate for patients and assist them in accessing necessary health and social services.
- Provide information, advice, and connections to community resources.
- Establish and maintain trust and rapport with patients and their families.
- Facilitate group workshops or activities as needed.
About You
- Active certification as a Community Health Worker in Texas, or equivalent experience.
- A holistic perspective of “health”. You will demonstrate the ability to work effectively in a wide range of settings with people from diverse backgrounds, including patients and co-workers.
- Effective written and oral communication skills.
- Comfortable working either one-on-one or in group settings; must possess excellent interpersonal skills.
- Active involvement in the community and a willingness to collaborate with others.
- Detail oriented - thoroughly documents patient encounter activities and follow-ups to resolve patient needs.
- Thinks critically; utilizes sound judgment; has ability to efficiently manage multiple tasks.
- Reliable transportation for occasional travel within the Houston metro.
- Basic computer skills and comfort learning new tools and systems required.
- Bilingual (English/Spanish) required.
- Knowledge/familiarity with the mental health and social service systems in Houston preferred.
