Performance Year Zero (PY0) represents the preparatory period that occurs before a new Aledade practice enters its initial performance year. During this phase, practices establish robust value-based care processes and complete implementation of technology-enabled solutions positioning them for optimal performance and early contract shared savings achievement.
The PY0 Performance Specialist supports the PY0 Performance team by providing project management capabilities and administrative coordination. This role focuses on market performance improvement through systemic project oversight, progress monitoring, and communication facilitation to ensure effective implementation of performance-driven strategies.
The Specialist serves as the primary escalation contact for practices encountering launch-related challenges, including practice engagement difficulties and technical implementation obstacles. The role involves troubleshooting these barriers and coordinating appropriate resolution pathways.
Additionally, the role contributes to the development and refinement of PY0 resources, supporting the creation of materials and tools that enhance the overall PY0 process.

Primary Duties:

  • Metric Monitoring and Escalation of Key Performance Metrics
  • Collaborate with the PY0 Performance leadership to review metrics and escalate concerns. Analyze KPIs (PY0 performance metrics, Milestone SLA adherence) to identify trends and bottlenecks. Prepare reports with findings and solutions. Engage stakeholders to gather insights and validate data. Facilitate cross-functional meetings to brainstorm and assign ownership for improvements. Monitor progress and provide feedback for continuous improvement.
  • Process Improvement and Workflow Development
  • Develop and update the market PY0 workflow, including the PY0 Market Playbook. Facilitate workflow integration into Salesforce/CRM.
  • Support the PY0 Performance leadership team in the organization and facilitation of stakeholder workgroup meetings, including developing agenda items, creating slides, and securing commitments from stakeholder teams.
  • Facilitate collaboration between the PY0 Performance leadership team, Product, Business Intelligence, Practice Success, and stakeholder teams to develop tools that enhance workflow and reporting for market and leadership needs.
  • Team Collaboration and Support
  • Collaborate with Customer Success and Marketing teams to develop field-facing learning resources for PY0, addressing needs identified by market teams.
  • Respond to inquiries regarding practice milestone blockers and launch questions, assisting Practice Transformation Specialists, Regional Medical Directors, and other market team leaders, and monitor communication channels for PY0 support needs.

Minimum Qualifications:

  • Bachelor’s degree in business, health administration, or public policy, or other relevant fields
  • 3+ years healthcare experience

Preferred Qualifications:

  • Experience with Salesforce/CRM
  • Flexible, self-starter, and comfortable working in a rapidly changing environment.
  • Project Management experience
  • Performance/Quality Improvement experience
  • Data analytic experience (pivot tables, Excel)
  • Ability to synthesize data from multiple sources to inform action plans and strategy.
