Position: Community Based RN (Salary/FT)

Summary: The AmeriHealth Caritas Health Plan (ACHP) Registered Nurse provides professional, patient centered nursing care in the primary care setting to all ACHD members under the supervision of the primary care provider. This ACHP RN will work with all teams including the Director of Quality, Clinical Manger, Chief Operating Officer, Outreach Manager, BCHC Management Team and others to develop processes and systems that operationalize BCHC’s mission and the ability to focus on preventative health. The ACHP RN identifies ACHP members with gaps in service or who may have cultural, logistic, or other care barriers. Provides face-to- face community-based support services to ACHP members to improve patient outcomes by keeping members connected to primary care, increasing member engagement in their own care, and identifying resources within the community to support these objectives. Schedules, coordinates provision of patient medical needs, plans care gap closures, and provides chronic condition education planning. Implements strategies that support effective quality care for all BCHC ACHP medical home patients through the use of the population health management system and care gaps closures. The ACHP RN actively supports all BCHC efforts to remain Federally Qualified Health Center (FQHC) and PCMH designation by providing quality care regardless of economic status.
