9amHealth is a Virtual Cardiometabolic Care company: a first of its kind, whole body approach to preventing and treating diabetes, high blood pressure, dyslipidemia, and obesity.
We provide members with a healthcare experience that includes virtual specialty physicians, at home lab tests, continuous glucose monitors, home delivered medications, and personalized meal plans. Our goal is to help our patients live healthier every day while maintaining affordability and the highest standard of care.
9amHealth was founded in 2021 by the team behind mySugr (acquired by Roche in 2017) and has brought together a team of leaders from across cardiometabolic healthcare along with the backing of leading investors like Cigna Ventures, 7Wire Ventures, and Founders Fund.
You must be able to work:
8:30 am-5:00pm PT - Tuesday-Saturday
OR
10:00 am-6:30pm PT - Sunday-Thursday
For you, caring for people with chronic conditions like diabetes and obesity isn’t a job, it’s a calling. You live and breathe it. Our Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions in collaboration with our Physicians. In the spirit of transparency, we anticipate hiring for this position in Q4.
Provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss care, education, and support for patients through chat, phone, and video calls following internal protocols
Create thorough consults aligned with internal protocols for 9amHealth physicians
Collaborate with Patient Care Advocates to ensure patient success
Minimum Qualifications
Registered Dietitian Nutritionist plus Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) or eligible for certification
Experience in medication adjustment and using an EMR
Strong empathy skills and an expert in patient-centered language and care
A quiet, distraction free dedicated work space
Preferred Qualifications
Experience with telehealth or using technology platforms in patient care
