9amHealth is a Virtual Cardiometabolic Care company — a first-of-its-kind, whole-body approach to preventing and treating diabetes, high blood pressure, dyslipidemia, and obesity.

We provide members with a healthcare experience that includes virtual specialty physicians, at-home lab tests, continuous glucose monitors, home-delivered medications, and personalized meal plans. Our goal is to help our patients live healthier every day while maintaining affordability and the highest standard of care.

9amHealth was founded in 2021 by the team behind mySugr (acquired by Roche in 2017) and has brought together a team of leaders from across cardiometabolic healthcare along with the backing of leading investors like Cigna Ventures, 7Wire Ventures, and Founders Fund.

What you’ll do

• Provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss coaching, education, and support for patients through chat, phone, and video calls following internal protocols

• Create thorough consults aligned with internal protocols for 9amHealth physicians

• Collaborate with RDs, PharmDs, and Patient Care Advocates to ensure patient success

• Support members in executing nutrition and lifestyle plans created by 9amHealth RDs for patients based on their medical conditions, goals, and preferences, ensuring they align with evidence-based guidelines for managing diabetes, hypertension, hyperlipidemia, and obesity

• Track patient progress through regular check-ins, assess the effectiveness of interventions and refer members to RDs, PharmDs, and providers to adjust care plans as needed based on clinical outcomes and patient feedback.

• Provide motivational interviewing, coaching, and counseling to support sustainable behavior changes in patients regarding diet, exercise, and stress management.

• Utilize patient data and health metrics to inform coaching recommendations, helping patients understand lab results and tracking outcomes like weight, blood pressure, and cholesterol levels.

• Maintain accurate and thorough patient documentation, ensuring all care actions, recommendations, and follow-ups are documented according to internal and legal standards.

What you’ll need

Minimum Qualifications

• An active health coaching certification or relevant certification (e.g., NBHWC, Wellcoaches, Duke Integrative, Diabetes Prevention Program (DPP) Lifestyle Coach certification, Certified Diabetes Care and Education Specialist (CDCES), or Board Certified-Advanced Diabetes Management (BC-ADM)

• Strong knowledge of and experience working with people who live with chronic cardiometabolic conditions, specifically diabetes, hypertension, hyperlipidemia, and obesity.

• Minimum 5 years of experience in direct client care, preferably in a clinical setting or via telehealth, with a focus on chronic disease management support.

• Experience using an EMR and other health coaching platforms.

• Demonstrated ability to build strong, trusting relationships with patients through compassionate, tailored communication.

• Experience working as part of an interdisciplinary team, with demonstrated ability to collaborate closely with physicians, patient care advocates, and other healthcare professionals.

• Experience providing care through virtual platforms (chat, phone, video), with comfort using digital health tools and patient management systems.

• A quiet, dedicated workspace free from distractions to ensure patient privacy and maintain confidentiality.

• Reliable high-speed internet connection to support phone, video, and messaging communications without interruption.

Preferred Qualifications

• Prior work at a digital health company focused on chronic conditions.

• Multilingual capabilities, particularly in Spanish, to support diverse patient populations.
